What happens without one
An AI receptionist hung up on the patient who needed it most.
As reported in August 2026, a 71-year-old stroke survivor in Armthorpe, near Doncaster, tried five times to book a GP appointment through EMMA, the AI phone receptionist her practice had installed. Her speech was fragmented after the stroke. The system asked her to repeat her name, could not parse it, and ended the call. It also asked callers not to use speakerphone, which she needed because one hand no longer worked a keypad. She registered with a different practice. The surgery has since decommissioned the tool and hired more reception staff. The vendor said a priority route straight to reception had been available all along. Nobody had switched it on.

The caller a model understands least is often the caller who needs a person most.
Speech models degrade on exactly the inputs that signal vulnerability: accents, impairments, distress. Low confidence on a voice line is a routing signal, not an error to retry. The fix is a confidence floor that hands the call to a human, built in before launch, not configured after the complaint.
The problem
Most voice failures trace to a missing fallback, not a weak model.
The model transcribed badly, which models do. The failure was that nothing sat between a low-confidence transcript and the decision to end the call. Gartner’s August 2026 survey of 3,566 customers found 87% consider a route to a human essential wherever generative AI handles service. The fix is a scoring layer that treats every low-confidence turn as a handoff trigger.
Source: Gartner press release, Aug 2026, survey of 3,566 B2B and B2C customers fielded Feb to Mar 2026The fix, conceptually
Listen first. Score every turn. Earn the right to act.
A voice agent should run beside the live line before it ever answers it, scoring what it would have said against what the human actually did. Once it goes live, every turn still gets scored: proceed, sample, review or hand off. The confidence floor for handing a caller to a person is a product decision made on day one, with the same weight as the greeting script.
The difference
Two front doors, one month apart, opposite outcomes.
Doncaster’s receptionist had a human route that was never enabled, so the system’s failure mode was a dropped call. Colorado Springs tested its police non-emergency agent for five months before launch, had human call takers review every AI-produced call, and hard-wired an immediate transfer to a person the moment it hears a disturbance or violence. Same category of technology. The difference was who designed the exit.
A voice agent without a handoff is a locked door with a greeting.
The Trust Gate
100% of every voice turn gets scored and logged.
Confidence above 94% proceeds automatically and is still logged. Between 85 and 94, a QA reviewer samples it. Between 70 and 85, a team lead reviews it. Below 70, a specialist takes the call before anything is promised to the caller. This is the same Trust Gate that runs inside BergAuto, and that Sirenn applies to SLA-bound voice workflows: bookings, dispatch, collections, front desk. BergFlow records the transcript, the score and the route for every call, so the audit trail exists whether or not a human ever had to pick up.
AI term explained
Shadow mode
- Definition
- Running an AI system in parallel with the live process, letting it generate every decision it would have made without allowing any of them to take effect, so its accuracy can be measured against real human outcomes before it is trusted to act.
- Why it matters
- Colorado Springs ran its non-emergency agent through supervised beta tests from April to September, with call takers reviewing every AI-produced call, before letting it answer first: shadow mode’s close cousin, measured before trusted. Deutsche Telekom’s AI chief describes evals and guardrails, not features, as the currency of a voice rollout. Doncaster’s practice went live without either, so the first measurement was a complaint. Every Sirenn deployment starts in shadow mode, and the Trust Gate thresholds are set from what the shadow run actually shows.
Spine first, then earn the rest
Start at 20% of calls. Earn the next 20 with evidence.
Deutsche Telekom is replacing phone menus with AI agents across ten European markets. Its chief AI officer, Kartik Sheth, put the curve on record in September: a new use case launches handling up to 20% of calls without a human, reaches about 40% within a couple of months, and every band above that gets harder. Simple intents first: a router reboot, a bill explanation. Complex, multi-issue calls stay with experienced people. Guardrails are set at the start and tightened as trust accrues, because adding them later is messy. That is the spine. Everything else is earned.
What it looks like done well
Colorado Springs put an AI agent on the police non-emergency line and cleared the queue.
In September 2026, Colorado Springs Police launched S.A.R.A.H., an Axon-built agent that answers the non-emergency line first. Non-emergency requests are about half of everything reaching the communications centre. Callers previously waited more than 30 seconds; the department now reports almost no wait. In four beta tests since April, roughly half of non-emergency calls were completed without a dispatcher. Anyone can ask for a person. The agent transfers immediately on any sign of violence or a disturbance. Human call takers review every AI-produced call. Annual cost: $170,000.

Every front door shares the same first move: answer, score, hand off.
Across the functions where a phone call is the first contact, the evidence from the last ninety days points at one spine: the moment the system decides whether a person is needed.

of non-emergency calls completed without a dispatcher in Colorado Springs beta tests; wait time from 30+ seconds to near zero. Colorado Politics, Sep 2026.

of customers say a route to a human is essential wherever GenAI handles service. One UK AI receptionist sits in front of 2M+ patients (vendor-stated). Gartner, Aug 2026; Digital Health, Sep 2026.
- Telecom care20% → 40%
of calls handled without a human, launch to month two, on Deutsche Telekom’s voice agents. Above 60% gets “really hard.” The Next Web, Sep 2026.
Where the spine lives: operations and back office
The same discipline decides whether the voice line ever pays back.
Further from the caller, the pattern holds. Most programmes cannot say what the voice layer returned, because nothing was measured before it went live.

- Collections93%
of collection agencies now use AI or machine learning, up from 49% in 2023. Live agent calls bring in under 60% of payments at most firms. TransUnion annual industry report, Feb 2026.

What the discipline adds up to
What that discipline adds up to.
“We do not put a voice on your phone line and hope the accent cooperates.”
Shadow run first, thresholds set from evidence, a human route that is live on day one, and a record of every call whether or not anyone had to pick up. One accountable system across strategy, platform, automation and people, not a vendor demo that sounded great in the boardroom’s accent.
Start here. Next issue, next month.
Your phone line is already the pilot. Run it in shadow before you run it live.
Sirenn wraps around the numbers you already publish: inbound, dispatch, bookings, collections, front desk. BergAuto scores every turn, BergFlow keeps the record, and our specialists take the calls the model should not. No rip and replace. BergLabs holds ISO/IEC 27001:2022 (Information Security Management System) and ISO 9001:2015 (Quality Management System) certification, and its data handling is aligned with GDPR and India’s DPDP Act, if legal is already reaching for that question. The next case in point and proof point land here next month.
